What is Family Connects Pierce County?



Whether you are a new or experienced parent, we all have unique needs when bringing a new baby into our family! Family Connects Pierce County is here to answer your questions and bridge the gaps between families, providers, and community resources to ensure you have the support you and your growing family need to grow and thrive.

As a Certified Family Connects site, we offer voluntary nurse support to all families with newborns during a baby’s first 3 months of life. This means foster, adoptive, and birthing families. An experienced nurse will support parents with a medical checkup for the birthing parent and baby, and ensure they are connected to community resources as they continue on their parenting journey.

We are committed to racial equity, and strive to provide inclusive, culturally sensitive postpartum care to every family we serve. All services are free of cost and offered to anyone in the community, when the program is fully implemented.

A Conversation Between Elevate Health and Family Connects Pierce County

Elevate Health’s Community Care Conversations bring together community leaders and health experts to tackle the health issues affecting our communities, and provides resources to improve your health and the health of your family. The Community Alignment Approach

Family Connects Pierce County is more than just a visit with a nurse. A major component of the model is understanding the needs of Pierce County families, and the capacity of service providers to meet those needs using a community alignment approach. Community Alignment is the process of working with community agencies, systems, and individuals to impact systems level change. No two families are identical – each will have their own goals, strengths, needs, parenting style, and cultural and language preferences. Family Connects Pierce County supports each family’s unique needs and preferences by utilizing an up-to-date directory of resources including the use of our local South Sound 2-1-1 Resource Directory. The goal is to improve the accuracy and efficiency of connecting families with the right service, at the right time, in the right amount. 

Data and Monitoring

Ongoing data collection and monitoring are critical components of the Family Connects model. Outcomes from the randomized control trial in Durham, North Carolina on the Family Connects Model produced the following results:

Eligibility Requirements for Pilot

We are currently serving a pilot population of families who live in 98499, 98404, 98408, or 98444, and who deliver at Tacoma General Hospital or St. Joseph Medical Center in Tacoma.
